About the Profession
What is Physiotherapy?
Physiotherapy is a healthcare profession focused on improving movement, reducing pain, and helping people stay as strong, mobile, and independent as possible. It can support recovery, function, balance, mobility, and confidence in everyday life.
Our community physiotherapists work with you to help you reach your personal goals and maintain them. This may include standing and walking more safely, moving around your home or community, improving strength and function, and managing daily activities such as dressing, showering, cleaning, getting out and about, and enjoying the things you love.
Conditions We Treat
How can physiotherapy help under Support at Home?
Our physiotherapists support older adults to stay safe, strong, mobile, and independent for longer through the Support at Home program. We provide physiotherapy that helps people age well at home, recover after illness or surgery, reduce falls risk, improve confidence and movement, and continue doing the everyday activities that matter most.
Support at Home is built around helping older people remain at home for longer, with services delivered using a wellness and reablement approach wherever appropriate.
Ongoing Services
Ongoing physiotherapy can support older adults who need regular help to maintain strength, mobility, balance, safety, and independence at home. This may include falls prevention, walking and transfer practice, pain management, strength and balance exercises, mobility support, and therapy to help people remain active and confident in their daily routines. Support at Home provides 8 ongoing funding classifications, with increasing levels of quarterly funding based on assessed need.
Restorative Care Pathway
The Restorative Care Pathway is designed for older people who would benefit from short-term, goal-focused support to maintain or regain function, reduce or prevent further decline, and improve independence after a setback in health or mobility. This is especially relevant for physiotherapy after surgery, hospitalisation, illness, falls, or a period of deconditioning. Under the program, the Restorative Care Pathway provides funding for up to 16 weeks.
Assistive Technology and Home Modifications (AT-HM) Scheme
The AT-HM Scheme supports access to assistive technology and home modifications separately from the participant's ongoing quarterly budget. For physiotherapy, this may include assessment and recommendations that support safer mobility, transfers, falls prevention, and function at home, alongside the use of appropriate equipment or environmental changes. The department states that AT-HM funding is accessed separately, usually for 12 months, with needs-based levels such as Low ($500), Medium ($2,000), and High ($15,000 or more in some cases).
Our physiotherapists can provide support across all of these areas, helping older adults maintain independence, recover well, and remain safe and active at home for as long as possible.
